The name SOAP - an acronym for Subjective, Objective, Assessment, Plan, - is a form of clinical documentation used by physicians to memorialize a patient encounter. It is estimated that some 15% of medical encounters result in a diagnostic error: diagnostic errors that result every year in nearly 400,000 lost lives, 400,00 disabilities, and over a trillion dollars in costs. The two most common errors are missed early cancers and heart diseases, which are also the two most common causes of death. The SOAP mission is to save the lives of millions of people by improving early disease detection and diagnosis.
